  • Employees of the Central/State Government and their families who are already covered under government medical reimbursement schemes are not eligible for coverage under DDSSY.

Documents Required for Deen Dayal Swasthya Seva Yojana (DDSSY)

The following documentation is required for enrollment in Deen Dayal Swasthya Seva Yojana (DDSSY):

  • Proof of residence for the eligible individual: This could be in the form of a Passport, Residence certificate, Election card, or Driving licence issued at least 5 years ago in Goa.

  • For individuals belonging to SC/ST communities: Caste certificate issued by the Director of Tribal Welfare or Deputy Collector.

  • For individuals belonging to the OBC non-creamy layer: Caste Certificate issued by the Deputy Collector along with an income certificate demonstrating an income of less than ₹6.00 Lakhs per annum.

  • For differently-abled individuals: Disability certificate issued by a Medical Board.

  • Aadhar Card or Enrolment Acknowledgement slip for every family member aged above 5 years (mandatory).

  • Ration card for family identification. If a ration card is unavailable, the following documents must be provided:

  • Marriage Certificate for the spouse.

  • Birth certificate or school leaving certificate for dependent children.

  • Declaration confirming that dependent parents are not employed.

Yearly Coverage Cap: The annual coverage cap for insurance benefits is as follows:**

  • Families with four or more members may receive coverage of up to ₹4 lakhs annually.

  • For families consisting of three or fewer members, the coverage may extend up to ₹2.50 lakhs annually.

  • The claim amount will never exceed ₹4.00 lakhs per year, ensuring financial protection against healthcare expenses.

Additional Coverage Option: Individuals have the option to enhance their coverage by opting for a higher level of insurance from the company upon payment of a higher premium. Significantly, this choice does not affect government policies and facilities.

Cashless Benefits: Beneficiaries of the scheme are entitled to receive healthcare benefits on a cashless basis, up to the yearly coverage cap.

Deen Dayal Swasthya Seva Yojana (DDSSY) can be registered through two modes. A detailed **Step-by-Step guide for both modes is provided below:

Online Application for Deen Dayal Swasthya Seva Yojana (DDSSY)

Step 01: Begin by visiting the official website of the Government of Goa and create an account to log in.

Step 02: Provide details of the eligible person or head of the family. Add details of all family members.

Step 03: Attach proof of residence, ensuring it covers a minimum of 5 years. Remember, all attached documents must be self-attested. Attach an Aadhaar card for each family member. For minors, attach a birth certificate if an Aadhaar card is unavailable.

Step 04: If you qualify for registration fee concessions, provide details of caste or income.

Step 05: Proceed to payment and pay the registration fees, structured as follows:**

For families of 4 or more members: Rs. 300/-

For families of 3 or fewer members: Rs. 200/-

  • A 50% concession is available with appropriate documentation for OBC non-creamy layer, SC/ST, and differently-abled eligible persons.

Step 06: After payment, note down the Acknowledgement Number. for tracking.

Step 07: The DDSSY e-card, Deen Dayal Health Card, will be available for download after the respective office verifies and approves your application.

Step 08: Upon approval, collect your DDSSY card within 30 days from the selected office during application entry.

Step 09: All communication will be sent to the registered mobile number.

How to Avail Benefits of Deen Dayal Swasthya Seva Yojana (DDSSY)

The recipient has the option to visit an Empanelled Health Care Provider (EHCP)

Following consultation with the physician, if the surgery is covered under the scheme and offered by the EHCP, the patient can approach the DDSSY Sahayak at the hospital with their Card.

The Sahayak will then initiate a request for pre-authorisation from the ISA pre-authorisation doctor. Alternatively, if the recipient wants to proceed with the treatment outside the state, here is the list of hospitals they can do the same.

Step 01: Begin by registering yourself on the official website of the Government of Goa. Provide your email address, mobile number, name, address, and date of birth, and authenticate using OTP.

Step 02: Once successfully registered, log in using your credentials.

Step 03: Navigate to the Services Menu, select All Services, then DDSSY, and finally, Deen Dayal Swasthya Seva Yojana (DDSSY) - Renewal.

Step 04: Enter your DDSSY Card Number and search for member details.

Step 05: Generate OTP to verify your identity. The OTP will be sent out to your registered mobile number.

Step 06: Fill in (*) mandatory fields if you are renewing with updates.

Step 07: Upload relevant documents if you are renewing with updates.

Step 08: Proceed to pay the relevant fees.

Step 09: After successful payment, make a note of the Acknowledgement Number for tracking purposes.
